Payment and Cancellation Policies

You agree to be financially responsible for all charges regardless of any applicable insurance or benefit payments. 

We do not bill directly to insurance.

Payment is expected at time of service unless you have made other payment arrangements with us.

You must provide 24 hours notice of cancellation/ reschedule, or a charge of $50 will be billed to the credit card on file.

No shows will always be charged the fee.

After your initial assessment appointment, you will be given a claim form to submit to your insurance

A pre-determination form will also be given to you to mail to your insurance if you wish to see if myofunctional therapy is covered under your insurance.

Depending on insurance, myofunctional therapy can be covered under dental, orthodontics and/or extended health benefits
